At SH Podiatry HOME VISITING SERVICE, I understand that personal information you provide is confidential. SH Podiatry HOME VISITING SERVICE is a member of the Information Commisioners Office, and all information is collected, controlled and processed in line with the General Data Protection Regulations (GDPR).
Personal information is collected at your initial appointment which is updated on subsequent visits. The information collected will be for no other reason than for your Podiatry medical record, to assist with diagnosis and treatment. This enables me to provide you with treatment that is as thorough and as safe as possible at every visit. Also enabling me to comply with legal and regulatory requirements by the Health and Care Professions Council, Royal College of Podiatry, and the Information Commissions Office.
Your Podiatry clinical records are stored securely in a locked cabinet and electronic devices used are password protected. Only accessible by myself – Sara Hutchings. Your personal details are not forwarded on to any third parties unless specific consent is obtained, for example a referral to a GP or another health care professional. If you provide information on behalf of someone else as an alternative contact, you can confirm that the other person has given their consent to process their personal data.
Retention of your Podiatry clinical records are for an adult, eight years after their last appointment. For children and those under eighteen until their twenty fifth birthday after their last appointment. All notes are shredded after the retention period.
